It is brought to the notice of all members that the Annual General Body Meeting of
The Speakasia Panelist Association of India (AISPA)
Will be held at VANMALI HALL, 1^st Floor, Opp Chabildas High School, Chabildas Road, Near Dadar Rly. Station, Dadar West, Mumbai 400 028 on Saturday, the 31^st March, 2012 at 10.00 am to transact the following business:
AGENDA

1.	Welcome Note and Address by the President.
2.	To elect the Office bearers of the Association for a three year term 2012-2015.
3.	To modify and amend the Rules and regulation of the Association.
4.	To appoint auditors for the Year 2012-2013.
5.	Any other business with the approval of the chair.
6.	Thank You Note by the Secretary.

Instructions for submitting the membership form

• Download the form.
• Affix your latest passport sized photograph.
• Please fill the Membership Form in all aspects clearly.
• Use BLOCK/CAPITAL letters while filling your name.
• Attach a Xerox of the Photo ID Proof. (Passport, Pan Card, Voter ID card)
• If the applicant is a certified Kohei, Senpei or Sensei please enclose the Xerox of the certificate.
• Membership Fees of Rs. 120/- per annum has to be paid by way of DD/PO issued by any Nationalized Bank Payable at Mumbai favoring Speakasia Panelist Association of India.
• Please do not staple the DD/PO to the form please use a pin or U clip.
• Please write the DD/PO Number behind your duly filled Form.
Send the duly completed form along with the Xerox of ID proof and DD/PO to:
